Background Minimally invasive, laparoscopically assisted surgerywas first considered in 1990 for patients undergoing colectomyfor cancer. Concern that this approach would compromise survivalby failing to achieve a proper oncologic resection or adequatestaging or by altering patterns of recurrence (based on frequentreports of tumor recurrences within surgical wounds) prompteda controlled trial evaluation.
Methods We conducted a noninferiority trial at 48 institutionsand randomly assigned 872 patients with adenocarcinoma of thecolon to undergo open or laparoscopically assisted colectomyperformed by credentialed surgeons. The median follow-up was4.4 years. The primary end point was the time to tumor recurrence.
Conclusions In this multi-institutional study, the rates ofrecurrent cancer were similar after laparoscopically assistedcolectomy and open colectomy, suggesting that the laparoscopicapproach is an acceptable alternative to open surgery for coloncancer.
