To the Editor: Plehn et al. (Nov. 26 issue)¹ present a case of primary (AL [amyloid light chain]) amyloid heart disease with atrial systolic failure. The authors provide no evidence of AL amyloidosis: they report neither plasma-cell dyscrasia nor monoclonal light chains in the plasma or urine, they give no immunochemical demonstration of AL proteins in amyloid deposits, and they do not exclude transthyretin, atrial natriuretic peptide, and AA (amyloid A) protein as components of amyloid deposits.

Although the authors describe the progressive pattern of diastolic dysfunction characteristic of amyloid heart diseases, they ascribe the absence of an "a" wave . . . [Full Text of this Article]
