To the Editor: We are concerned by the conclusion of Ramsey et al. (June 17 issue)1 that aerosolized tobramycin is not only efficacious but also safe. They state that there was no significant difference with regard to the emergence of tobramycin-resistant bacteria between regimens of aerosolized tobramycin and placebo. During their study tobramycin-resistant Pseudomonas aeruginosa infection developed in 10 of 71 patients (14 percent), P. cepacia in 3 (4 percent), and P. maltophilia (Xanthomonas maltophilia) in 10 (14 percent).
