To the Editor: Endothelin, a newly isolated, endothelium-derived peptide, has contractile and proliferative effects on vascular smooth-muscle cells1,2. Plasma endothelin concentrations are elevated in patients with pulmonary hypertension3. Pulmonary-artery pressure is increased in patients with mitral stenosis. We hypothesized that increased endothelin production in the pulmonary circulation might have a role in the increased vascular tone in this disorder. We therefore measured plasma levels of endothelin-1 in blood from the right atrium and left atrium of 10 patients with mitral stenosis who were undergoing percutaneous mitral valvuloplasty.
