Presentation of Case

A 49-year-old man was admitted to the hospital because of myelofibrosis with myeloid metaplasia and painful osteolytic lesions of the right femur.

He had had essential thrombocythemia for 10 years that evolved into myelofibrosis with myeloid metaplasia. The hematologic findings throughout the patient's course are presented in Table 1. Hydroxyurea was administered, with a moderate decrease in the platelet count. Three years before entry massive splenomegaly was detected.
