A 27-year-old man was admitted to the hospital because of syncope.
The patient had been in excellent health until the day of admission, when he experienced palpitations and dizziness while playing soccer and abruptly lost consciousness. He awoke after two or three minutes and was unaware of chest pain or dyspnea. He was admitted to the hospital.
The patient was born in El Salvador and was raised in an agricultural area in a small house without plumbing. At the age of six years he had a severe illness with considerable loss of weight; he was given herbal remedies and recovered.
