The United States is moving forward with health care reform in an unplanned fashion that reflects its belief in entrepreneurial capitalism. Steps are being taken in two areas toward an era in which doctors, hospitals, and other health care providers will combine to form a variety of new organizations. In the private sector, payers and providers are channeling patients into managed-care plans, a growing number of which are controlled by large insurance companies1.
