To the Editor: An outbreak of tuberculosis resistant to rifampin, isoniazid, streptomycin, and ethambutol occurred in our institution1 in association with conversions among our health care workers after tuberculin skin testing. Sixteen workers entered a six-month investigational trial of ofloxacin (800 mg a day) and pyrazinamide (1500 mg a day), after approval by the appropriate review boards.

Adverse effects due to these drugs were very common. Of the 16 workers, 14 discontinued prophylaxis after less than six months; 13 workers had one or more adverse effects, including arthralgia (7 workers), gastrointestinal distress (6), hepatitis with alanine aminotransferase levels of 491 . . .
