Chest discomfort and negative U waves in precordial leads V, V, and V of the electrocardiogram (black arrows) with T-wave inversion occurred after the infusion of dipyridamole in a patient with severe stenosis of the proximal left anterior descending coronary artery. These findings occurred concomitantly with severe myocardial hypoperfusion in the anteroseptal region during a thallium stress test. The area of hypoperfusion subsequently filled in (white arrows).
