To the Editor: O'Brien et al. (Feb. 3 issue)1 reported a low rate of remission after 2-chlorodeoxyadenosine therapy among patients with chronic lymphocytic leukemia refractory to fludarabine. However, 57 percent of the patients had a decrease in the lymphocyte count of 50 percent or more. The study confirmed that patients who have already received extensive therapy are at high risk for thrombocytopenia when treated with 2-chlorodeoxyadenosine. In our experience, thrombocytopenia due to 2-chlorodeoxyadenosine is no more common among patients who have received fludarabine than among patients who have received other therapy,2 and it may also develop in previously untreated patients3. . .
