A 64-year-old man presented to the emergency room with fever, chills, nausea, malaise, and left-leg pain that had lasted for four days. For the past two days he had been unable to lift his left leg.

I assume he has been unable to lift his left leg because of pain. Acute pain that limits the ability to move a leg makes me think about a vascular catastrophe, trauma, or a fracture. We don't have a lot of history, except a nonspecific febrile illness that may or may not involve the gastrointestinal tract.
