Presentation of Case

A 77-year-old woman was admitted to the hospital because of pain in the head and legs, fever, and sweats.

The patient had been well until four weeks earlier, when she began to experience pain in the neck that extended over the lower third of the left side of the head and left mandible. The next morning she had an acute onset of "burning" in the lower legs, as if they were "on fire." That night she changed her nightgown several times because of sweats.
