Presentation of Case

A 38-year-old woman was evaluated in the clinic because of severe rheumatoid arthritis and a new left pleural effusion.

The patient was a hairdresser. She had had seropositive rheumatoid arthritis for 8 1/2 years. On repeated tests the rheumatoid factor ranged between 522 and 1440 IU per milliliter, and a test for antinuclear antibodies was negative; the erythrocyte sedimentation rate ranged between 40 and 69 mm per hour. Repeated physical examinations were negative except that the patient was overweight and had extensive rheumatoid articular deformities and rheumatoid nodules that were most prominent about the hands and elbows.
