A 52-year-old man was brought to the hospital because of lethargy and headache. He had a history of mild, intermittent headaches and heartburn but had had worse-than-usual occipital and left retro-orbital pain for the past one or two days. There was no trauma, photophobia, nausea, or vomiting, and his only medications were pseudoephedrine and aspirin, which he had taken that morning. He also described several recent bouts of heartburn in his left chest and shoulder that were independent of exertion, position, and ingestion of food and that had resolved spontaneously after 5 to 10 minutes.
