In this issue of the Journal, Muto et al.1 report a decrease in the incidence of second primary hepatocellular carcinomas in patients treated with polyprenoic acid, an acyclic retinoid, after the resection or ethanol ablation of small, solitary primary hepatocellular carcinomas. The rationale for this clinical trial was based on studies in animal models of hepatocarcinogenesis and hepatoma cell lines, which suggested that polyprenoic acid inhibits chemically induced or spontaneous hepatocarcinogenesis and suppresses cell growth and the production of alpha-fetoprotein in human hepatoma cell lines.
