A 50-year-old black man was brought to the emergency room. He had been unable to move his legs for four days and complained of impaired speech.

The lesion in this patient may be in the lower-cranial-nerve area, perhaps due to a basilar meningitis, or somewhere in the cerebral hemispheres. I assume that it does not appear septic, because this was not described as his presenting picture. He appears to have a global process that is affecting his brain stem rather than higher cortical function, since he is not described as confused. What additional information do we have?
