In this issue of the Journal, two papers address the role ofradiation therapy in patients with breast cancer treated withadjuvant chemotherapy. These prospective, randomized trialsfrom Denmark1 and Canada2 present 15-year results. Althoughthis long follow-up is necessary for a proper evaluation, thequestion the studies ask and the results they present must beinterpreted in terms of the current practice of breast-cancermanagement. Stating the purpose of the studies explicitly isuseful, but to appreciate the implications of the results fullywe need to consider the hypotheses of tumor pathogenesis thatunderlie the therapeutic maneuvers being tested.
