In response to the controversy over placebo-controlled trialsin developing countries of treatments to prevent mother-to-childtransmission of the human immunodeficiency virus (HIV),1 I wouldlike to offer the perspective of someone who lives and worksin Thailand, a developing country with limited resources thatis the site of several such trials.

I believe that placebo-controlled trials are sometimes justifiedin countries where treatment is otherwise totally unavailable,because at least half the patients (those receiving active treatment)will probably benefit.
