In response to the controversy over placebo-controlled trials in developing countries of treatments to prevent mother-to-child transmission of the human immunodeficiency virus (HIV),1 I would like to offer the perspective of someone who lives and works in Thailand, a developing country with limited resources that is the site of several such trials.

I believe that placebo-controlled trials are sometimes justified in countries where treatment is otherwise totally unavailable, because at least half the patients (those receiving active treatment) will probably benefit.
