To the Editor: Lai et al. (July 9 issue)1 provide important data on the clinical and virologic efficacy of lamivudine for the treatment of hepatitis B virus (HBV). However, because of the potential for mutations of the gene encoding the RNA-dependent DNA polymerase of HBV, it is very unlikely that a single medication will ever become standard therapy for this infection.

We are particularly concerned about the choice of a once-daily regimen of lamivudine. The anti-HBV activity of lamivudine yielded 50 percent inhibitory concentrations of about 0.01 µM (i.e., values nearly identical to those reported for the human immunodeficiency virus . . .
