To the Editor: Metformin is an effective treatment for many patients with type 2 diabetes mellitus. Although it lowers serum vitamin B₁₂ concentrations, it does not cause anemia.¹ We describe a patient with metformin-induced hemolysis and jaundice.

A 46-year-old man presented at our hospital on March 31, 1998, with a one-week history of polydipsia and polyuria. The physical examination was normal except for the presence of obesity (body-mass index [the weight in kilograms divided by the square of the height in meters], 29.4).
