We describe the reappearance of HBV infection 10 years after renal transplantation in a 42-year-old immigrant from Singapore who was believed to be immune to the virus. Before and after transplantation, she was consistently negative for hepatitis B surface antigen (HBsAg), positive for antibodies to HBsAg (anti-HBs) and core antigen (anti-HBc), and negative for HBV DNA on polymerase-chain-reaction (PCR) testing.1
