Figure 1. A 36-year-old woman who had been a vegan for many years presented with a one-week history of decreased visual acuity. She had had megaloblastic anemia six years earlier (hemoglobin, 7.5 g per deciliter) that was found to be due to folic acid deficiency. At that time, her serum level of vitamin B12 was normal, as were the results of a Schilling test. There was no evidence of malabsorption, and an x-ray study of her small bowel was normal.
