To the Editor: In his article on support for academic medical centers, Iglehart (July 22 issue)1 seems to describe the issue of variation in Medicare's payments for residency training as a conflict among states, with New York defending the current policy and California and Texas promoting a change. In fact, this is not an issue of New York versus California and Texas.
