To the Editor: Kannus et al. (Nov. 23 issue)1 report that the use of external hip protectors reduces the risk of hip fracture in elderly people by 60 percent. We believe that there is uncertainty about the magnitude of the effect because the analysis was not performed on an intention-to-treat basis. The authors excluded 298 subjects who declined to participate after randomization, 68 percent of whom had been assigned to the hip-protector group.
