A 67-year-old man with a history of excessive alcohol consumption was admitted for evaluation of dyspnea on exertion, palpitations, and lightheadedness. He had had a motor vehicle accident 13 years earlier that resulted in multiple compound fractures of his left leg. Right heart catheterization revealed a resting cardiac output of 10 liters per minute and a clinically significant increase in oxygen saturation in the inferior vena cava (oxygen saturation, 93 percent). An arteriogram revealed a massive arteriovenous malformation arising from the left internal iliac artery.
