An 18-year-old woman (gravida 0, para 0) presented with a six-month history of abdominal distention but no history of weight loss or abnormality in menstruation. Radiography showed an abdominal mass with a ground-glass appearance and displacement of bowel into the upper half of the abdomen (Panel A). An abdominal computed tomographic scan revealed a huge, well-encapsulated mass occupying the abdominal cavity (Panel B). Exploratory laparotomy revealed a large cystic mass arising from the left adnexa.
