To the Editor: Cluster headache is the most severe form of primaryheadache.1 Positron-emission tomography has shown activationof the homolateral posterior inferior hypothalamic gray matterduring attacks of cluster headaches, a finding that is apparentlyspecific to the condition,2,3 and voxel-based morphometric magneticresonance imaging (MRI) has documented alteration of the samearea,4 suggesting that cluster headache may be initiated inthis area. We reasoned that stereotactic stimulation of thisarea might prevent activation and relieve intractable formsof cluster headache.
