Profound vasoconstriction in the peripheral circulation is thenormal response to conditions in which the arterial pressureis too low for adequate tissue perfusion, such as acute hemorrhagicor cardiogenic shock. In other conditions, the most frequentof which is septic shock,1,2 hypotension occurs as a resultof failure of the vascular smooth muscle to constrict. Suchso-called vasodilatory shock is characterized not only by hypotensiondue to peripheral vasodilatation but also by a poor responseto therapy with vasopressor drugs.
