The National Emphysema Treatment Trial (NETT), reported in thisissue of the Journal (pages 20592073) raises interestingquestions about the interpretation of secondary analyses ofdata collected in clinical trials. After exclusion of 140 patientswith a high risk of death from surgery according to an interimanalysis,1 the 538 patients who were randomly assigned to surgerywere more likely than the 540 assigned to medical therapy tohave improvements in exercise capacity and quality of life,but there was no reduction in mortality during an average of29 months of follow-up.
