To the Editor: Heeschen et al. (March 20 issue)1 show that troponin T and soluble CD40 ligand independently predict the outcome in patients with acute coronary syndromes and that the increased risk conferred by elevated CD40 ligand levels is reduced by abciximab. They show that elevated levels of soluble CD40 ligand predict adverse outcome even in patients who are negative for troponin T, but they do not indicate whether patients who are troponin T–positive and have low levels of soluble CD40 ligand have an increased risk of adverse events, or whether this group also benefits from treatment with abciximab.
