To the Editor: Hayes and Piesman (June 12 issue)1 state that an important strategy for preventing the late-stage manifestations of Lyme disease is the early diagnosis and treatment of erythema migrans. In Europe, in contrast to North America, a special type of late-stage borreliosis involving the skin sometimes becomes manifest years after a tick bite.2 After an untreated inflammation, the extremities may become involved, with advanced atrophy of the skin (which appears like crumpled cigarette paper with visible veins).
