The dramatic increase in the incidence of type 2 diabetes mellitusand obesity in the Western world signals the beginning of anepidemic that is expected to become a major cause of illnessand death. At the heart of this problem lies insulin resistance,the earliest detectable abnormality in the development of diabetes.Many states give rise to insulin resistance, and all are explainedby numerous mechanisms in which insulin signaling is decreased.Du et al.1 have recently described another, involving a proteinthey call TRB3 that specifically blocks the actions of insulinin the liver.
