To the Editor: On the basis of the results of their clinical trial, Bjornson and colleagues (Sept. 23 issue)1 reach two conclusions that are not supported by their data. First, the authors "advise cautious use of dexamethasone in children with recent exposure to varicella or preexisting immunodeficiency." Since they specifically excluded such children, their study offers no basis for this recommendation. Second, they "advocate dexamethasone treatment for essentially all children with croup," presumably on the basis of the modest benefits of treatment and the absence of serious adverse events among 359 subjects in the dexamethasone group.
