To the Editor: Antiobesity operations, particularly the Roux-en-Y gastric bypass, are increasingly common. In the United States, 103,000 such operations were performed in 2003.1 We report a complication of a gastric bypass procedure that resulted in the death of a pregnant woman and of her fetus.

At 31 weeks' gestation, a 41-year-old woman (gravida 1) presented at her community hospital with midepigastric pain, nausea, and vomiting that had begun 30 minutes after she had ingested a fatty meal.
