During the past 15 years, there have been important advancesin both percutaneous coronary intervention (PCI) and the pharmacotherapyof acute coronary syndromes without ST-segment elevation. Severalcarefully designed, randomized clinical trials have evaluatedprospectively two contrasting strategies for managing thesesyndromes (a routine early invasive strategy and a selectivelyinvasive strategy) and their effect on short- and long-termclinical outcomes. With the early invasive strategy, all patientsundergo early coronary angiography, followed by PCI if the coronaryanatomy permits.
