To the Editor: Hudes et al. (May 31 issue)1 suggest the use of temsirolimus as first-line treatment for metastatic renal-cell carcinoma. On the basis of their trial, temsirolimus was the second new drug to receive Food and Drug Administration (FDA) approval, after sunitinib,2 in that setting. We think that temsirolimus should be the preferred drug in patients with metastatic renal-cell cancer and a poor performance status. The authors' conclusions, however, do not account for subgroups, and the FDA allows the use of both temsirolimus and sunitinib as first-line therapy. How can physicians choose between the two treatments?
