To the Editor: With regard to the editorial by Lewis (July 19 issue),1 the "perfect clinical trial" for Crohn's disease should be generalizable and should predict effectiveness and the benefit-to-risk ratio in practice. Lewis acknowledges that the remission rate at 6 weeks (absolute Crohn's Disease Activity Index [CDAI] score, ≤150) was low (22% among patients receiving certolizumab pegol vs. 17% among patients receiving placebo, P=0.17) at 26 weeks in the Pegylated Antibody Fragment Evaluation in Crohn's Disease: Safety and Efficacy 1 (PRECISE 1) trial.2 However, there is insufficient discussion of the prediction of effectiveness in practice with the primary end . . .
