A 52-year-old man with cardiomyopathy and interstitial lung disease was admitted to the hospital in extremis.
Asthma began at the age of 10 years. A coarctation of the aorta was repaired at the age of 22 years. At the age of 36 years progressive exertional dyspnea developed, with orthopnea and paroxysmal nocturnal dyspnea. Furosemide was prescribed, with improvement.
At 37 years of age the patient was admitted to this hospital for cardiac catheterization. He consumed one or two bottles of wine and smoked 1 1/2 packs of cigarettes daily. On physical examination the blood pressure was 120/80 mm Hg. The
