Presentation of Case

A 71-year-old man was admitted to the hospital because of a rash, diffuse muscle weakness, and respiratory failure.

There was a long history of insulin-dependent diabetes mellitus and alcoholism. The patient had been in stable health until two or three months before entry, when a diffuse maculopapular erythematous rash developed. It improved with prednisone therapy but flared when the dose was tapered, with the onset of difficulty in walking and arising from a chair. Twenty-six days before admission he experienced sudden right flank pain and was admitted to another hospital.
