Presentation of Case

First admission. A 73-year-old man was admitted to the hospital because of worsening right upper facial pain and decreasing visual acuity in the right eye.

There was a history of Hodgkin's disease localized to the right groin 27 years earlier, which was treated with excisional biopsy and radiation treatment, without a recurrence. More recently, the patient required treatment for primary hypothyroidism, insulin-dependent diabetes mellitus, hypertension, chronic atrial fibrillation, and chronic obstructive pulmonary disease; he had a long history of cigarette smoking until three years before entry. Six years before admission chronic lymphocytic leukemia (CLL), stage 0, was diagnosed.
