A 24-year-old woman in the 21st week of her first pregnancy was admitted to the hospital with severe leg edema. Three years earlier, after arthritis and a facial rash had developed, a diagnosis of systemic lupus erythematosus had been made, and she had been treated with hydroxychloroquine. Laboratory tests at that time showed thrombocytopenia and anti-DNA antibodies. A year earlier the patient had had pure red-cell aplasia that was unresponsive to high-dose steroids but responsive to intravenous immune globulin.
