A 72-year-old man was sent to the emergency room by a visiting nurse, who had found him at home in respiratory distress. He had a two-week history of malaise, anorexia, dyspnea, and chest pain.

My primary concern is to determine the cause of the respiratory distress. The illness is subacute with a two-week prodrome. The antecedent malaise and the anorexia are consistent with a viral illness, but the dyspnea and chest pain also make me think about a cardiac process. At this time I don't have enough information.
