To the Editor: We wish to highlight a shortcoming of the study by Packer et al. (July 1 issue)1 regarding the withdrawal of digoxin from patients with chronic heart failure treated with angiotensin-converting-enzyme (ACE) inhibitors. The authors state that during the single-blind run-in period the doses of ACE inhibitor were increased "so that a daily dose of at least 25 mg of captopril or 5 mg of enalapril was achieved."
