To the Editor: Cockcroft et al. (April 14 issue) report preserved endothelium-dependent vasodilatation in response to carbachol and acetylcholine in patients with essential hypertension1. In repeated studies, however, my colleagues and I have consistently found a depressed response to acetylcholine in patients with hypertension, a finding reproduced by several other investigators, even in a study of human arteries in vitro2. More recently, we have demonstrated that such impaired vascular responses also extend to other endothelium-dependent agonists, such as substance P3 and bradykinin4.
