Presentation of Case

A 74-year-old woman was admitted to the hospital because of diarrhea with recurrent dehydration and severe weight loss.

There was a history of an "irritable colon" for many years, manifested by intermittent diarrhea that usually lasted one or two days and resolved spontaneously. Evaluation elsewhere was reported to show lactose intolerance; a hiatus hernia with reflux esophagitis and peptic ulcer disease were shown on fiberoptic esophagogastroduodenoscopic examination seven months and four months before admission, with symptomatic improvement after the administration of omeprazole; an upper gastrointestinal series was said to reveal antral and pyloric scarring with gastric dilatation.
