Non-insulin-dependent diabetes mellitus (NIDDM), the commonestform of diabetes, affects approximately 5 percent of the populationof the United States. Although the pathophysiology of this conditionhas not been fully characterized and the nature of the primarydefect is controversial, the results of recent studies provideinsights into its basic causes. Here, we review these studiesand suggest how insulin resistance and genetically programmedpancreatic beta-cell dysfunction may interact in susceptiblepersons to cause diabetes.
