To the Editor: In their article "A Reversible Posterior LeukoencephalopathySyndrome," Hinchey et al. (Feb. 22 issue)1 described severalcases of a condition that is clinically and radiographicallysimilar to hypertensive encephalopathy2,3 and the related syndromeof cyclosporine-induced neurotoxicity.4 These disorders havepreviously been well characterized.2,3,4
Several points in the paper require clarification. Of greatestconcern are the methodologic flaws in the study design thatled the authors to conclude that the syndrome is always reversible.They retrospectively selected only patients with white-matterabnormalities who subsequently recovered from their neurologicevent.
