A 67-year-old woman had a 20-year history of Paget's disease of bone, resulting in conductive hearing loss, bone and joint pains, pathologic fractures, limb deformities, and difficulty walking. Treatment with calcitonin and etidronate had been unsuccessful. Panel A shows anterior and posterior images from a bone scan obtained after the administration of technetium-99mlabeled methylene diphosphonate. The markedly increased accumulation of radionuclide in almost the entire skeleton, sparing only the distal extremities, reflects the increased bone turnover in Paget's disease.
