To the Editor: O'Dell et al. (May 16 issue)¹ addressed the important topic of optimal therapy for rheumatoid arthritis. However, we are concerned that the authors' conclusions — "in patients with rheumatoid arthritis, combination therapy with methotrexate, sulfasalazine, and hydroxychloroquine is more effective than either methotrexate alone or a combination of sulfasalazine and hydroxychloroquine" — may have been overstated.

The authors studied only patients who had poor responses to various drugs, including methotrexate, hydroxychloroquine, and sulfasalazine, the drugs used in the study.
