Presentation of Case

A 37-year-old man with the acquired immunodeficiency syndrome (AIDS) was admitted to the hospital because of dyspnea and wheezing of three months' duration.

Nine years earlier, a test for antibodies against the human immunodeficiency virus (HIV) had been positive. Five years before admission, a tuberculin skin test (purified protein derivative, 5 TU) was positive, and isoniazid was administered for a year. Fourteen months and 10 months before admission, he was treated for Pneumocystis carinii pneumonia and thereafter received prophylactic doses of dapsone.
